Sign and date your application in blue or black ink.
THIS IS REQUIRED! You can only apply for an absentee ballot with a form that contains your original signature.

Mail (do not fax) the completed application to:

The application must be mailed to the Board of Elections not later than the 7th day before Election Day, or delivered to the Board of Elections not later than the day before Election Day.
